Appointment Letter

1. Appointment Letter Information Page

Complete applicable data on this screen and press Create Appointment Letter. An edit screen will appear with the required language and your particular details as entered; the letter is then fully updateable.

Top Margin  - The width that you would like the top margin to be. Used to adjust reports for different letterheads. Only the first page of the letter will be adjusted. All other pages will be set to one inch

Salutation Line (include comma) - **REQUIRED. Free-form field as what to address the reader.(e.g. Dear Ms. Smith,; Dear John Smith,; etc.)

Employee Address (Y/N) - **REQUIRED. 'Y' or 'N' if you want to include the employees address on the letter. Defaults to 'Y'.

FTE % (Y/N) - **REQUIRED. 'Y' or 'N' if you want to include the FTE % after the title. Defaults to 'Y'.

Hourly Pay Rate - **REQUIRED. Will automatically fill in if the 'Selected' applicant was appointed in IADS.

Employing Unit Name - **REQUIRED. Name of the employing unit to which the applicant is appointment. Defaults to the UDDS division and department name.

Length of Probationary Period (months) - Number of months to be served as the probationary period (Can be blank or a fraction).

Who to Report to (Name)  - Name of the person to report to on the first day.

Who to Report to (Date) - First day to report for the new position. Defaults to the appointment begin date from IADS.

Who to report to (Time) - Time of the day to report for the new postion. Defaults to 8:00a.m.

Contact for I9 Form (Name) - Person to contact to obtain I9 Form.

Contact for I9 Form (Phone) - Phone number of the person to contact to obtain I9 form.

Contact for Disability Accomodations (Name) - **REQUIRED. Person to contact to get information about disability accomodations.

Contact for Disability Accomodations (Phone) - **REQUIRED. Phone number of the person to contact to get information about disability accomodations.

Contact for Insurance Forms (Name) - Person to contact to obtain insurance forms.

Contact for Insurance Forms (Phone) - Phone number of the person to contact to obtain insurance forms.

Person Signing (Name) - Name of the person signing the appointment letter. Prints toward the right side of the letter.

Person Signing (Title) - Title of the person signing the appointment letter. Prints toward the right side of the letter.

CC's - List of names that are to get copies of the appointment letter (does no special processing, just puts the names at the end of the letter).

Create Appointment Letter - Creates appointment letter using specified data and opens the Appointment Letter Edit screen for user modification.

Close - Closes the Appointment Letter screen.

2. Appointment Letter Edit Page

IMPORTANT: The text box is fully updateable. Be careful not to remove any of the required language.

Any changes made in the text box will be carried over to the printed version of the report. If changes are made in the text box, they will not be carried over to the information page. If the Recreate Default Appt Letter button is pressed without saving your changes, any changes will be lost.

Print Appointment Letter - Press button to Print the Appointment Letter.

Save - Saves the Appointment Letter in case it needs to be accessed and changed at a later time. (If the appointment letter is saved, each time the appointment letter is choosen on the Print Reports screen for this vacancy, the Appointment Letter Edit Page will be the frst page to display.) NOTE: The saved appointment letter only applies to the associated vacancy.

Recreate Default Appt Letter - Returns to the Appointment Letter Information screen.

Close - Closes the Appointment Letter screen.
